You need to apply to the Imperial Household Agency. Online application form can be found on this page: http://sankan.kunaicho.go.jp/order/index_EN.html , and this is the official address of the location: http://www.kunaicho.go.jp/eindex.html . It is open monday to Friday from 8.45 am-noon & 1-4 pm. They have offices located on the west side of the Imperial Park, near Imadegawa subway station.
